Family Planning/Birth Control/Sexually Transmitted Infections

Clinic Information:

Clinic appointments can be made by calling 719-583-4380.  Family Planning and birth control services are free for individuals under the age of 20. For those who are age 20 and older, charges are based on a sliding fee scale that is dependent upon your income and how many people you support. Sexually transmitted infection testing and treatment will be associated with low-cost fees.

Monday & Wednesday
9:00am - 3:00pm

Tuesday, Thursday & Friday
8:00am – 4:30pm

Extended Hours:
The third Thursday of every month until 6:00pm

Medical Records 

Patients requesting a copy of Medical Records can print the Authorization to Release Information From PDPHE Clinic Form, complete it and bring it to the Pueblo Department of Public Health and Environment.

  • Contraception Supplies (Pills, Patch, IUD, Depo, Condoms, Implanon, and Plan B)

  • Cancer Screenings (Pap Smears and Breast Exams)

  • Well-Woman Exams

  • Pre-Pregnancy Counseling

  • Sexually Transmitted Infections - Testing and Treatment

Confidential Services Available for Men:

  • Pregnancy Planning and Prevention Education

  • Contraception Supplies

  • Emergency Contraception

  • Sexually Transmitted Infections – Testing and Treatment
